Sign Up Form Instructions

Open the form and save it to your computer but hitting save as.

 Fill out the form by tabbing to each area.

Some boxes are check boxes, but for days and times, there is a drop down box to choose 1 2 or 3

Please identify whether you are a small group leader,

If so, where you intend to hold the small group,

And again, the times that are best for you.

Any other comments you think are necessary should also be included.

Save the document again to your computer.

Mark Subject as DIM. 

I will reply with a confirmation that your sign-up has been received.

Once all groups are formed, I will contact you again with group specifics such as

Day of Week

Time of Day

Group Leader

Location

Directions on how to get there, if at someone’s home.
